Tissue Viability Resources

Tissue Viability Clinical Nurse Specialist referral process: 

1. Complete ‘TVN Referral’ form in patient/resident’s case notes
2. Notify Tissue Viability Clinical Nurse Specialist by email tissueviability@rhn.org.uk
3. When completing wound assessments, use ‘NURSING Wound Assessment’ form in case notes. Where patients/residents have multiple wounds, a separate form should be completed for each wound. Wound must be assessed at least every 7 days.

Note: Urgent medical problems must be referred to responsible medical team during hours (Monday-Friday 9am – 5pm), or the Clinical Response Service out of hours.
