Patient Safety Incident Response Framework

The Patient Safety Incident Response Framework (PSIRF) was published in August 2022, outlining how providers should respond to patient safety incidents for the purpose of learning and improvement.

To improve our approach to how we respond to patient safety incidents, we have begun a 12-month period of preparation ahead of transitioning to NHS England’s new Patient Safety Incident Response Framework.

The framework sets out new guidance on how organisations respond to patient safety incidents, and ensures compassionate engagement with those affected. It focuses on understanding how incidents happen, rather than apportioning blame; allowing for more effective learning, and ultimately safer care for patients.

All staff are required to complete Level 1a Essentials of Patient Safety. All managers must also complete Level 2 Access to Practice.
