RHN End of Life Care Model

Historically the RHN uses the terms Palliative Care, End of life Care and Terminal Care (Care of a dying person) interchangeably and this invariably leads to confusion. The updated RHN model explains and clarifies these terms. Please refer to it and print out the model here for easy access.

It also details suggested conversations (and documents) which, if started, open up important discussions around end of life issues. This is ultimately about finding out what is important to that individual and family.
