Requesting New Forms or Form Changes on PatientSource

We have a new and improved system for requesting new forms or changes to forms in PatientSource. We will no longer accept requests via email.

To request a new form or any changes to an existing form, please use the new link called ‘Patient Source Form Requests’ on the Intranet under Clinical Resources.

Your request will then be approved by the Health Records Group to help ensure that we are always improving our documentation and help to prioritise the request amongst all the other requests. This will help to streamline the work leading to things being done more efficiently for everyone!
